He first appears listed together with Zhang Rang, Zhao Zhong, and seven others as the "Ten Attendants," described as "banding together for evil" c:5801. In the struggle between the two palaces of Empress Dowager Dong and Empress Dowager He, Duan Gui and Zhang Rang and the others, by bribing He Jin's younger brother He Miao and his mother Lady Wuyang, were able to continue receiving the protection of Empress Dowager He, thereby gaining a firm footing in the political struggle c:2594.
When He Jin summoned outside troops into the capital intending to execute the eunuchs, Duan Gui and Zhang Rang and the others plotted to strike first. They contrived to lure He Jin into the palace, set armed assassins in ambush before the gate of the Jiade Hall, and finally killed He Jin c:2595. After He Jin was killed, the capital fell into chaos, and Yuan Shao, Cao Cao, and others led troops into the palace to seize and kill the eunuchs. Duan Gui and Zhang Rang and the others, in their desperation, abducted the young Emperor Liu Bian and the Prince of Chenliu, Liu Xie, and fled by night through the smoke and fire toward Mount Beimang c:2596. On the road of flight, however, they were pursued by Min Gong, a subordinate official of Nanzhong. Seeing the situation become critical, Zhang Rang in his despair drowned himself in the river. The novel does not clearly state Duan Gui's final fate, but as he fled together with Zhang Rang and Zhang Rang then drowned himself, Duan Gui's destiny is also broken off here, implying that he too may have perished or disappeared in this episode, never appearing again.
